The therapist provides rehabilitative and preventive services to residents and outpatients, including taking history, systems review, examination, evaluation, prognosis, and diagnosis; develops treatment plan and discharge plans; provides appropriate interventions and reevaluation/adjustment of treatment plans as required. The therapist participates in rehabilitation education for Trinity staff, the community, and other organizations as needed. The therapist may be required to consult to contracted facilities. The therapist may participate in committees such as quality assurance, corporate compliance, or others, as assigned.
MINIMUM QUALIFICATIONS AND REQUIREMENTS
Licenses and Certifications Required
Current ND physical therapy license.
Educational Requirements
Graduate of accredited physical therapy program.

Basic computer skills and word processing skills required.
